As Robyn Williams, a 27 year old woman from Rensselaer, learned the hard way a number of doctors are charging to complete the DB450 form which requires employers to provide coverage for temporarily disabled workers.  The form must be submitted to the employer's disability insurance carrier to verify the need for benefits.  Not all doctors are charging to complete this paperwork.  While the fee is allowed the Workers' Compensation Board, who produces the form, has no way of knowing which doctors charge to complete it and which do not.  The board does not administer the temporary disability program which is for injured workers with illnesses and injuries that are not sustained on the job, and usually only becomes involved in a case if there is a dispute about whether a disability benefits claim is valid.  Read full article.
